All NEW DOGGIE DOMAIN boarding and day care guests must complete ALL of the following steps of the MEET & GREET interview process:

COMPLETE Part A of Vaccination & Health Certification-Form #1 and Background Information-Form #2. |

SUBMIT VACCINATION AND HEALTH CERTIFICATION FORM- #1 to your vet to complete and sign Part B. |
| 3. |
FAX (480-862-7733) completed/signed Form #1 + Form #2 to DOGGIE DOMAIN. |
| 4. |
CALL(480-862-7722) to verify receipt of forms, and SCHEDULE the 15-20 minute "Meet & Greet Interview". |
| 5. |
ATTEND "Meet & Greet Interview". During this visit, your dog(s) will become acquainted with our living staff , accommodations and doggie pals. In addition, all NEW DOGGIE DOMAIN BOARDING GUESTS will schedule a half day of daycare prior to checking in for their boarding stay. |
